Becoming a new client is now easier than ever!

All new clients are required to complete the intake form on this page and
authorize a credit card (not debit card) to keep securely on file.
The card must be active and valid. Your card will only be charged in the event of
a cancellation policy violation or for any unpaid final balance.

Please note: The person submitting this form must be the client and owner of the hairpiece being sent in for service. Service agreements and policies can only be accepted by the client directly.
While payment may be made by a third party (such as a spouse or family member), the client is
still the responsible party for compliance with all service policies. If a third-party card is used,
the cardholder must provide separate authorization for billing.
Submissions from businesses, wig brands, or other third parties are not accepted.
