All new clients are required to complete this intake form and authorize a credit card (not debit) to be kept securely on file.
Your card must be active and valid. It will only be charged if there is:
a cancellation policy violation, or
an unpaid final balance after services are complete.

The person submitting this form must be the client and owner of the hairpiece being sent in for service.
Service agreements and policies must be accepted directly by the client.
While payment may be made by a third party (such as a spouse or family member), the client remains responsible for compliance with all service policies.
If a third-party card is used, separate cardholder authorization is required.Submissions from businesses, wig brands, or other third parties are not accepted.
